Procurement of Surgical Supplies

REQUEST FOR QUOTATION SVP – 26 – 249 Tondo Medical Center, through its Bids and Awards Committee (BAC), intends to procure the project Procurement of Surgical Supplies with an Approved Budget for the Contract of One Million Nine Hundred Eighty-Six Thousand Nine Hundred Sixty-Three Pesos (PhP 1,986,963.00) through Section 34 Small Value Procurement of the Implementing Rules and Regulations of Republic Act No. 12009. Please submit the following documentary requirements addressed to the BAC Chairperson on or before 09:00 AM of 16 October 2026 at procurement@tmc.doh.gov.ph or bac@tmc.doh.gov.ph, subject to compliance with the Instructions and Terms and Conditions provided on this Request for Quotation (RFQ): 1. Duly accomplished and signed Technical Specifications using the attached form; 2. Duly accomplished Price Schedule using the attached form; 3. Current Valid Certificate of PhilGEPS/ PhilGEPS registration number; Should your company be declared as the Single/Lowest Calculated Bidder (S/LCB), the following must be submitted within five (5) calendar days from the receipt of Post-Qualification Notice: 1. Mayor’s Permit / Business Permit; 2. BIR Certificate (2303); 3. SEC or DTI Certificate; 4. Income/Business Tax Return; 5. Notarized Omnibus Sworn Statement with Secretary’s Certificate or Special Powers of Attorney, whichever is applicable 6. Valid Certificate of Medical Device Registration (CMDR) / Certificate of Medical Device Notification (CMDN) / Certificate of Exemption issued by the Food and Drug Administration (FDA), whichever is applicable; • Valid License to Operate (LTO) issued by the FDA for Suppliers, Distributors and Traders; • Product samples for evaluation Product samples shall be labeled with the following details:  Name of Company  IB Number and Name of the Project  Item Number  Item Description Tondo Medical Center reserves the right to reject any and all quotations, declare a failure of procurement, or not award the contract in accordance with Section 70 of the IRR of RA No. 12009.
