SUPPLY OF LABOR, MATERIALS, TOOLS, EQUIPMENT & SUPERVISION FOR PMS OF NISSAN URVAN (PLATE NO. SKB 1538) SCOPE OF WORKS: 1. PERFORM PMS ACTIVITY FOR THE FOLLOWING: A. ACTIVITY NO.1 - 40,000 KM B. ACTIVITY NO.2 - 50,000 KM TERMS AND CONDITIONS: 1. This PMS must be completed within ONE (1) DAY per activity. 2. The interested bidder shall submit cost estimates and bill of materials of work activities during opening of bids for evaluation. 3. NPC shall not be held liable for any untoward incident that may happen during the course of the job. 4. Provide accomplishment report prior to acceptance. 5. The commencement of work will start upon the issuance of PO and Notice to Commence depending on the mileage of the vehicle. 6. The winning bidder shall submit a sworn statement that his/her workers conform to the following requirements. a. At least 18 years old above b. Has not been involved in any liability, case, crime, or record covered by the PNP/NBI and its affiliates. 7. The winning bidder shall submit photo with corresponding dates.
