As a government agency, Bongabon Water District shall deal only with legitimate suppliers/contractors who issue BIR-registered Official Receipts. 2. All entries must be typewritten. 3. Quoted prices shall be inclusive of applicable taxes and shall be firm and valid for a period of at least thirty (30) days from the date receipts of quotation. 4. Delivery of materials must be within seven (7) days upon receipts of Notice of Proceed. 5. Price Quotation must be attached with Warranty Certification for all items stating warranty period of not less than one (1) year and item must be replaced with brand new if found/become defective during operations. 6. BWD reserve the right to post-qualify any supplier and/or to reject any all submitted quotations. 7. Valid PhilGEPS Registration Certificate-Platinum Member, DTI/SEC Registration, Current Mayor’s/Business Permit, Updated General Information Sheet (GIS), Annual Income Tax Return/Business Tax Return paid thru EFPS shall be accepted, and Tax Clearance Certificate must be submitted with the quotation. 8. All documentary requirements must stamp “Certified True Copy” and signed by representative. 9. Payment shall be on staggered basis. 10. Approved Budget for the Contract – Php1,085,000.00 11.
