Republic of the Philippines City of Lipa BIDS AND AWARDS COMMITTEE REQUEST FOR QUOTATION FOR SMALL VALUE PROCUREMENT Repair and Maintenance of ICT Equipment of the City Engineer's Office SVP-2026-10-015 The City Government of Lipa through its Bids and Awards Committee (BAC) intends to undertake a SMALL VALUE PROCUREMENT for the project, ''Repair and Maintenance of ICT Equipment of the City Engineer's Office '' in accordance with Section 34 of the Implementing Rules and Regulations of Republic Act No. 12009 or the New Government Procurement Act (NGPA). Name of Project: Repair and Maintenance of ICT Equipment of the City Engineer's Office Approved Budget for the Contract: ₱95,995.00 Source of Fund : General Fund Appropriation 2026 All interested suppliers must submit the accomplished and signed Quotation Form on or before October 19, 2026, 8:00 AM. subject to the compliance with the Terms and Conditions provided on this Request for Quotation (RFQ). Interested bidders shall also submit copies of the following documents along with the duly signed Quotation on or before the above specified deadline of submission of quotation: a. PhilGEPS Registration Number/ Certificate b. Valid Mayor’s / Business Permit The Supplier/Service Provider with the Single/Lowest Calculated Quotation shall submit its Omnibus Sworn Statement (NGPA GPPB-prescribed forms), and Income/Business Tax Return (for ABC’s above Php 500,000.00) within a non-extendible period of five (5) calendar days from receipt of the notice that it submitted the Single/Lowest Calculated Quotation. The Head of the Procuring Entity (HOPE) of the City of Lipa reserves the right to reject any and all quotations, declare a failure of procurement, or not award the contract in accordance with Section 70 of the IRR of RA No. 12009.
